Nomination Form

Thank you for taking the time to nominate your favorite youth sports team or athletic association for the Adirondack Fleece Get in the Game campaign!

Please be as complete as possible as we will be relying on your nomination form to complete their entry into the current month’s contest. If you have contact information it would be extremely helpful to have in the event your organization wins so that we can obtain the necessary logos to get their blankets printed promptly.

Also, if you are planning to nominate a large, national organization, please be sure to note if you are nominating a specific chapter or local office. Our contest rules dictate that we can only profile an organization once every six months, so we want to be sure if it is a specific chapter or local office that other nominations for different chapters or offices of the same organization can be permitted in subsequent contests.

We will be posting a profile of each entrant received within a week of the date that the completed nomination form is received by Adirondack Fleece. For complete details on the campaign, please visit our Official Rules page.

You will receive an email from us confirming that we are in receipt of your nomination. The date of the post on your nominee will determine which month’s contest that they will be entered for a chance to win.
